据了解,前屯村常住人口400人左右,大多都是60岁以上老人。老年人是慢性病的高发人群,村里有141人患有高血压、糖尿病等慢性病。

2023年,魏县建成县乡村三级慢病管理体系,县医院牵头搭建全县慢病管理平台,安排专家下沉坐诊,乡镇卫生院负责日常管理和随访,村卫生室负责入户筛查和健康教育。截至目前,慢病管理平台共管理全县慢性病人群超11万人,系统内管理对象高血压达标率从52%提升到65%,糖尿病达标率从48%提升到59%。

在第十三届县域卫生发展大会上,魏县的“23611”慢病管理模式被国家卫生健康委向全国推广。
